A key example is the recent scandal at Wells Fargo, where the bank admits creating up to 3.5 million potentially fraudulent accounts costing many retail banking customers money and impacting their credit ratings. Although the bank has settled some claims against it, regulatory actions and litigation are on-going. How did this happen? How can similar problems be prevented in the future?

Marineau, a former CEO of Fortune 500 companies and currently a partner at a private equity firm; Juliana S. OReilly, an executive and managing counsel for American Express; and Carol Ann Petren, a former top executive and general counsel for CIGNA Corporation.
